The Division of Developmental Disabilities Health Home (DD Health Home) is being designed in collaboration with the Missouri Department of Social Services, MO HealthNet Division. The DD Health Home is intended for individuals statewide served through the Division of DD, who have a qualifying chronic health condition, or are at higher risk of health issues or destabilization. The DD Health Home provides population health management and assists individuals with meeting their health and wellness goals while also ensuring individuals get the care they need.

If you are interested in DD Health Home services and believe you are eligible for the program, please complete a self-referral form.
