The Division of Developmental Disabilities Health Home (DD Health Home) was designed in collaboration with the Missouri Department of Social Services, MO HealthNet Division. The DD Health Home is intended for individuals statewide served through the Division of DD, who have a qualifying chronic health condition, or are at higher risk of health issues or destabilization. The DD Health Home provides population health management and assists individuals with meeting their health and wellness goals while also ensuring individuals get the care they need.
- Eligibility
Individuals eligible for Division of DD services and covered by MO HealthNet are eligible to be served by a DD HH if they have a chronic condition of Intellectual and/or Developmental Disability; and have or be at risk of developing one of the following conditions:
- Diabetes;
- Asthma;
- Cardiovascular disease or hypertension;
- Chronic Obstructive Pulmonary Disease (COPD);
- Overweight (Body Mass Index (BMI)>25);
- Dementia;
- Dependent on a ventilator;
- Tobacco use;
- Diagnosis of Autism Spectrum Disorder;
- One of the Fatal Five Plus Conditions: Pulmonary Aspiration, Bowel Obstruction, Gastroesophageal Reflux Disease (GERD), Seizures, Sepsis, Dehydration;
- Using the Health Risk Screening Tool that identifies potential risk for individuals with a Healthcare Level of 3 or greater.

Forms must be submitted to the Division of DD at DDHHEnrollTransferDischarge@dmh.mo.gov. Please encrypt all emails containing PHI by using the State of Missouri’s Proofpoint System.
- Enrollment
The Division has identified approximately 20,000 individuals statewide, both waiver and non-waiver, who are eligible for the DD Health Home program. Auto-enrollment began in June 2024, for DD Health Home services starting July 1, 2024. The rollout is occurring in phases until April 2025:
First Quarter Auto Assignment for DD Health Home services starting July 1, 2024:
- 5,655 eligible individuals received enrollment letters dated June 14, 2024
- Regional Office Areas: Central, Springfield, and St. Louis.
Second Quarter Auto Assignment for DD Health Home services starting October 1, 2024:
- 6,092 eligible individuals will receive enrollment letters in mid to late September 2024.
- Regional Office Areas: Central, Hannibal, Joplin, Poplar Bluff, Sikeston, Springfield, and St. Louis
Third Quarter Auto Assignment for DD Health Home services starting January 1, 2025:
- An estimated 4,959 eligible individuals will receive enrollment letters in December 2024.
- Regional Office Areas: Joplin, Kansas City, Kirksville, Poplar Bluff, Rolla, Sikeston, and Springfield.
Fourth Quarter Auto Assignment for DD Health Home services starting April 1, 2025:
- An estimated 3,511 eligible individuals will receive enrollment letters in March 2025.
- Regional Office Areas: Albany, Joplin, Kansas City, Rolla, and Springfield.
