DD Health Home

The Division of Developmental Disabilities Health Home (DD Health Home) was designed in collaboration with the Missouri Department of Social Services, MO HealthNet Division. The DD Health Home is intended for individuals statewide served through the Division of DD, who have a qualifying chronic health condition, or are at higher risk of health issues or destabilization. The DD Health Home provides population health management and assists individuals with meeting their health and wellness goals while also ensuring individuals get the care they need.

Anyone may submit a DD Health Home Referral to determine if an individual is eligible for the program.

Eligibility

Individuals eligible for Division of DD services and covered by MO HealthNet are eligible to be served by a DD HH if they have a chronic condition of Intellectual and/or Developmental Disability; and have or be at risk of developing one of the following conditions:

Diabetes;
Asthma;
Cardiovascular disease or hypertension;
Chronic Obstructive Pulmonary Disease (COPD);
Overweight (Body Mass Index (BMI)>25);
Dementia;
Dependent on a ventilator;
Tobacco use;
Diagnosis of Autism Spectrum Disorder;
One of the Fatal Five Plus Conditions: Pulmonary Aspiration, Bowel Obstruction, Gastroesophageal Reflux Disease (GERD), Seizures, Sepsis, Dehydration;
Using the Health Risk Screening Tool that identifies potential risk for individuals with a Healthcare Level of 3 or greater.
