What is Health Risk Screening Tool (HRST)?

The Health Risk Screening Tool (HRST) is a tool used to provide early detection of health risks and destabilization.  The HRST tool is a component of the Missouri Division of Developmental Disabilities Health Risk Screening (HRS) Process that has been implemented statewide to support waiver participant health and welfare assurances.

Overview

NOTE: The Division issued Provider Bulletin #32 along with a HRST Transition Plan on February 9, 2024. The HRST Transition Plan will allow the Division and stakeholder’s additional time to develop a HRST process that can be achieved successfully. As such, the timeline included in the posted in the HRST Process Phase II document will be adjusted accordingly.

Rating Service Information

To assist residential service provider agencies during a reported Residential Oversight RN vacancy, the Division has contracted with IntellectAbility (IA) to provide agencies assistance to complete HRST screenings and subsequent Health Risk Support Plans (HRSPs). To receive the HRST Rating Service, the agency will need to complete the REDCap RN vacancy form located via the Provider Portal.
