Projects for Assistance in Transition from Homelessness (PATH)

Projects for Assistance in Transition from Homelessness (PATH) is a federal formula grant created under the Stewart B. McKinney Homeless Assistance Act. The grant supports outreach to individuals with serious mental illness, as well as individuals with co-occurring substance use disorders, who are homeless or at risk of becoming homeless in all 50 states, District of Columbia, Puerto Rico and four U.S. Territories.
