Disqualification Registry

The Disqualification Registry contains the names of individuals who are disqualified from working with consumers receiving services from the Department of Mental Health. The lists consists of individuals with a substantiated charge of:

  • Physical abuse
  • Sexual abuse
  • Class I neglect
  • Misuse of client funds or property
  • Two Class II neglect or verbal abuse charges within a 12 month period.

Individuals that desire in-home services for a child, senior or person with disability may request a screening through the Department of Mental Health's Disqualification Registry by completing the Caregiver Background Screening form
