Behavior Resource Team - Criteria for Intervention

• The Planning Team has identified that the individual's behavior puts him/her at risk of losing  placement, dismissal from a job or school, etc.
• The Support Team has met and tried options to appropriately address behavioral concerns.
• The Planning Team has identified that the individual has behavioral support needs and is moving to new placement due to behaviors.
• An individual is moving into residential placement from a habilitation center.
